Death Certificates
Important Notice: The Southern Nevada Health District Vital Records office has received an increased number of calls about third-party vendors selling birth and death certificates. These companies charge customers high fees to “process” or “help apply” for certificates issued from the Department of Health. If you have ordered through one of these third-party companies and would like to file a consumer complaint, please call the State of Nevada Attorney General Bureau of Consumer Protection at (702) 486-3132 or toll free at 888-434-9989. SNHD Vital Records sends all birth and death certificates via First Class Mail through the United States Postal Service (USPS). SNHD Vital Records is not responsible for any lost, misdirected, or undelivered mail by USPS or any other delivery service.
Ordering a Death Certificate
Death certificates can be requested online, by mail, or dropped off in office.
Valid photo identification of the requestor and proof of relationship if required must be included with all applications. Incomplete applications will not be processed.
If a record is not found, a non-refundable $10.00 search fee may be applied.
Qualified Applicant must have direct and tangible interest (NAC 440.070)
NAC 440.070 Issuance of certified copies of certificates. (NRS 440.120, 440.650)
- As used in NRS 440.650, “direct and tangible interest” means a:
- Direct relationship by blood or marriage within the second degree of consanguinity to the person named on the certificate.
- Legal relationship to the person named on the certificate.
- Requirement imposed by law or otherwise to facilitate legal process; or
- Funeral director or his or her designee from the funeral home who is listed on the death certificate within 180 days after the date of registration of the death.

Payment will be requested by email after the request has been reviewed, please allow 2-3 business days.
SHIP TO/MAIL requests will arrive via first class mail.
PICK UP:
- Your order will be available for pick up once payment has been made.
- Pick up location: 280 S. Decatur Blvd., Las Vegas, NV 89107
- Pick up hours: Monday–Thursday 7 a.m.–5 p.m.
- Certificates using the pickup option will be held for 60 days from the date payment was made.
- Certificates will only be released to the requestor.
Drop Off
Personal Check are not accepted
Using the mail application, drop off the completed Death Certificate Application with your Money Order (payable to SNHD) and all required documents in the drop box found in the office lobby. Incomplete requests will not be processed.

Costs
To waive the $13 registration fee (attached to the purchase of the first certificate), proof of prior payment must be verified. The $13.00 registration fee will not be refunded once payment is made.
