Clinics
All Nevada school children must be immunized. The health district offers extended hours and special Saturday shot clinics in August and September.

Costs
Vaccine costs vary based on the type of immunization required. In addition to the vaccine costs, the health district charges an administrative fee of $16 per child for one vaccine and $25 per child for two or more vaccines.
Payment by Visa†, MasterCard†, cash, Culinary Insurance*, Nevada Check-up, and Medicaid is accepted. Not all vaccines are covered by insurance.
† Credit cards are not accepted at offsite clinics, such as malls, community centers, etc.
*Culinary insurance is only accepted for children less than 19 years of age.
Note: The credit card holder's name must match the name on the acceptable photo identification presented.

Immunizations Required for Students
Nevada law requires that all children be fully immunized before entering school.

Vaccine Administration Record
Download the vaccination administration record in English or Spanish. PDF 47KB. The form must be completed in ink, signed and brought to the middle school clinic in order for your child to be vaccinated.
